BIS Forms and How to order them

The Bureau of Industry and Security stocks the various forms required to comply with the Export Administration Regulations. Generally, these forms are only available through the U.S. mail, they cannot be faxed or sent by e-mail. BIS Form 711, however, is available in PDF format on this site. All Bureau of Industry and Security forms are available free of charge. Limited quantities are available from BIS's Western Regional Office in California, and most Department of Commerce's, U.S. and Foreign Commercial Service District Offices.

Please note that on October 20, 2008, electronic filing using the SNAP-R system became mandatory for Export and Reexport License Applications, Classification Requests, Encryption Review Requests and License Exception AGR Notifications.
